Policies

Your child has been referred to our clinic to be seen by a pediatrician. Please note that this referral does not mean that you are a primary patient for this physician. A Pediatrician is a specialist and generally does not take on primary care of well children. We recommend all our patients have a family doctor.

  • The referral provides you a consultation with a pediatrician to review and assess your child. The physician will determine what followup will be required.

  • You must follow up as instructed by the Pediatrician, if you do not comply with their recommendations or it has been more than 12 months since you were last see, you will be required to be reassessed by your family doctor. If they feel the need for you to be seen again a referral may be sent to our clinic.

  • For ongoing care patients, if it has been more than 12 months since your last visit, you need to have your family doctor send a new referral letter. You will not automatically be able to return as ongoing care patient. This status will be at the Doctors discretion.

  • Once the physicians feels your child’s issues have been addressed, they may decide to direct you back to your family doctor for primary care.

  • Your child MUST be present for all appointments.

Missed Appointments

Our office expects all patient to adhere to the proper practice for cancelling and rescheduling appointments. There are many children waiting for appointments and the staff requires adequate time to fill an open time slot. There are penalty fees for patients who do not provide enough notice for cancelling/rescheduling appointments.

Follow up appointments require a minimum of 24 hours notice. Without proper notice the fees are as follows:

  • 15 minute appointments - $50.00

  • 30 minute appointments - $100.00

All consults require a minimum of 48 hours notice. Without proper notice the fees are as follows:

  • 60 minute appointments - $300.00

  • 90 minute appointments - $350.00

Running Late?

If you are running late (5-10 mins) please contact our office to inform us. Please note that if you are more than 15 mins late for your appointment you may be asked to reschedule your appointment.

Uninsured Fees

Prescriptions

The physicians do not prescribe/renew prescriptions for patients without being seen prior. If the prescription has been lost the physician is able to fax/call in a prescription. The fees associated to that are:

For standard prescriptions - $35.00

For triplicate prescriptions - $45.00

** If you are unable to pay the above prescription fees you will need to book an appointment to be able to receive the prescription**

Copy of Chart

For requests of chart copies for another physician and/or the family will be subject to the chart copy fee. Please note that we only provide chart copy via USB. The fee for the chart is $35.00.

Letters/Forms

If you require a letter or form filled out by the physician be aware that there could be a fee. The fee depends on the type of letter or form requested.
